Doctor’s first name and last name
Patient’s first name and last name
Doctor’s email address

You may optionally include notes and a scanned copy of the prescription (Rx) with your uploaded files. Patient data is collected solely to perform health care operations.

Usage Data

Usage Data is collected automatically when using the Service.

We may obtain payment details including credit card data which is stored on our encrypted in house servers. Additionally, that information may be provided to Our third-party payment processors whose use of Your personal information is governed by their Privacy Policy. Invisible reCAPTCHA

We use an invisible captcha service named reCAPTCHA. reCAPTCHA is operated by Google.

JotForm

We use a HIPAA-compliant form-building service named JotForm to facilitate uploading shade images, radiograph images, and digital impression files. You must include the doctor and patient’s first and last name and the doctor’s email address when uploading files. We automatically delete uploaded files and their accompanying information 30 days after we have processed them.

“Do Not Track” Policy as Required by California Online Privacy Protection Act (CalOPPA)

“Do Not Track” (DNT) is a web browser setting that instructs a web application to disable its tracking of a particular user. Our Service does not respond to DNT signals.

Children’s Privacy

Our Service does not address anyone under the age of 13. We do not knowingly collect personally identifiable information from anyone under the age of 13.
