PHI

While using Our Service, You may choose to upload certain PHI such as images and digital scans. We require the following information to accompany the files you upload:

  • Doctor’s first name and last name
  • Patient’s first name and last name
  • Doctor’s email address

You may optionally include notes and a scanned copy of the prescription (Rx) with your uploaded files. Patient data is collected solely to perform health care operations.

Usage Data

Usage Data is collected automatically when using the Service.

Usage Data may include information such as Your Device’s Internet Protocol address (e.g. IP address), browser type, browser version, the pages of our Service that You visit, the time and date of Your visit, the time spent on those pages, unique device identifiers and other diagnostic data.

When You access the Service by or through a mobile device, We may collect certain information automatically, including, but not limited to, the type of mobile device You use, Your mobile device unique ID, the IP address of Your mobile device, Your mobile operating system, the type of mobile Internet browser You use, unique device identifiers and other diagnostic data.

To manage Your requests: To attend and manage Your requests to Us.

Email Marketing

We may use Your Personal Data to contact You with newsletters, marketing or promotional materials, and other information that may be of interest to You. Payments

We may process payments or provide other transactional services within the Service. In that case, we use third-party services for payment processing (e.g. payment processors).

We may obtain payment details including credit card data which is stored on our encrypted in house servers.  Additionally, that information may be provided to Our third-party payment processors whose use of Your personal information is governed by their Privacy Policy.   We currently utilize CardConnect to process payments.  Their Privacy Policy can be viewed at https://cardconnect.com/privacy-policy

Usage, Performance and Miscellaneous

We may use third-party Service Providers to provide better improvement of our Service.

Invisible reCAPTCHA

We use an invisible captcha service named reCAPTCHA. reCAPTCHA is operated by Google.

The reCAPTCHA service may collect information from You and from Your Device for security purposes.

JotForm

We use a HIPAA-compliant form-building service named JotForm to facilitate uploading shade images, radiograph images, and digital impression files. You must include the doctor and patient’s first and last name and the doctor’s email address when uploading files. We automatically delete uploaded files and their accompanying information 30 days after we have processed them.

“Do Not Track” Policy as Required by California Online Privacy Protection Act (CalOPPA)

“Do Not Track” (DNT) is a web browser setting that instructs a web application to disable its tracking of a particular user. Our Service does not respond to DNT signals.

Children’s Privacy

Our Service does not address anyone under the age of 13. We do not knowingly collect personally identifiable information from anyone under the age of 13. If You are a parent or guardian and You are aware that Your child has provided Us with Personal Data, please contact Us. If We become aware that We have collected Personal Data from anyone under the age of 13 without verification of parental consent, We take steps to remove that information from Our servers.
