Year-End Tax Forms


Sign up for paperless year-end tax forms in Paycom by navigating to: Payroll > Year-End Tax Forms. This is also where you will find your W-2 and 1095-C (if applicable) once they are finalized.

A paper copy of Form 1095-C is available upon request only. To request a paper copy of your form, please contact Cassie Rugh at cassie@obriendentallab.com; 4311 SW Research Way, Corvallis, OR 97333; or 541-512-4255. Forms will be provided within 30 days of request.

Group Supplement Insurance - Accident, Critical Illness, and Disability

  • Allstate supplement plans have been acquired by The Standard.

  • Learn more about your coverage plans.

  • Access forms or file a claim online.

401k Retirement Plan

  • Invest in your retirement by enrolling in O’Brien’s 401k Plan.
